Sign into your JotForm account.
Design a new custom form from scratch with our drag-and-drop Form Builder, or select one of our 10,000+ templates.
Go to the Settings tab in the top navigation panel, and then select Integrations on the left side of the page.
Select the Zendesk App, enter your Zendesk subdomain, and click Authenticate. You will then be prompted to log into your Zendesk account.
Once your Zendesk account has been authenticated, you can match your form fields to your Zendesk fields.
The form fields Name, Email, Subject, and Description are required in order to generate a support ticket.
Map additional form fields to custom ticket fields, select a group, and add any tags.
If a form contains an upload field, this file can be attached directly to the support ticket.
Click the Save button to complete the integration.
That's it! When your form receives a new submission, it will automatically be sent to Zendesk as a new support ticket.
